Demographic data form for service users
It's important to be sure your project is reaching everyone it should be, using a demographic data collection form is a good way to encourage people to provide this important information. Remember Demographic information is needed for your monitoring reports and can help you encourage future funding.
You only ask the data questions that make sense for your project, but as a minimum try and collect age, race and sex.

Sign-in sheets
Best for evets, a sign in sheet allows you to collect very basic data that can help you to get a snapshot of your users at any one event or session. It is also helpful to help you build an email contact list if a registration isn't right for you.

Photographic and video consent form
If you want to use a photo of an individual(s) for published and promotional use you should always get consent to do so under UK GDPR legislation and have a policy on how you will store and manage these pictures. In public spaces, while it is generally legal to take photos without consent - at a festival for example - care should still be taken as individuals do still have privacy rights depending on the context. We advise you to do your research to be sure your don't fall foul of GDPR regulations.
Useful tip - Use large or coloured stickers for people that don't give their consent, so the photographer can be sure not to take or use that persons picture.

For larger events or activities, especially working with more vulnerable communities it is a good idea to undertake a risk assessment. This highlights the issues you have and helps you to think about mitigation needed to reduce the risks.
